Ego dystonic sexual orientation

A 39 years old Muslim male teacher from a lower middle class family of rural background was admitted in psychiatry ward accompanied by his family members with history of sudden brief jerky movement of all four limbs and body while fell into sleep, diificulty  in falling asleep, poor appetite and generalized weakness, headache, intolerance to noise, decrease interest in activities for about five years. He gave history of passage of semen in urine, night emission in dreams for ten to 12 years. Sexual orientation and behaviour (attraction and inclination towards male sex as well as anal intercourse with several males with associated guilt feeling) leading to divorce within one and half month of marriage. Premorbid gloomy mood without psychiatric illness in family. Mental status finding of preoccupation with chronic ill health, ego dystonic sexual orientation (conflict between performance and religious teaching), level four insight. Psychometry showing mild depression with passive faminine role, dependent character. Hamilton depression scale scoring 17. Provisional diagnosis was ego dystonic sexual orientation (F66.1) with Dhat syndrome (F48.8) with moderate depressive episode with somatic syndrome (F32.11).
