Paranoid schizophrenia, continuous

S.R., a 28 years old unmarried Hindu male was admitted with chief complaints of some discomfort in head and limbs for two to three months and as per informant, talking out of context, muttering and smiling to self, reduced sleep with wandering out, disorganized and aggressive behaviour, and irritability for two years. The symptoms were sudden in onset. Patient had undergone treatment from Psychiatry outdoor patients' department (OPD) of Silchar Medical College Hospital for two months, a month after the onset of symptoms. Then after three months of onset of his symptoms, he allegedly murdered his mother. Patient reported of a feeling of fearfulness about people trying to harm him for one and half yeras. Along with this, he also said of hearing voices till seven to ten days prior to hospital admission. His younger sister had an episode of psychotic illness with complete remission. Premorbidly he had an extrovert type of personality with poor moral values and religiosity. Mental status examination revealed depressd mood, restricted and inappropriate affect, low volume monotonous speech, without any disturbance of thought and perception presently, concrete thinking and level three insight. Provisional diagnosis was paranid schizophrenia, continous (F20.00).
